We provide comprehensive, low cost health services to residents of all ages in Northwest Colorado. Our Community Health Centers, Prevention Services and Home Health and Hospice programs address important health needs from birth through end-of-life. We provide services on a sliding fee scale based on income; we do not deny health care due to financial limitations. Our services include primary care, behavioral health, dental health, family planning and sexual health, Health Communities and Communities That Care.

Additional Information

The Healthy Communities Program helps families with children under age 21 and pregnant women apply for Health First Colorado, Colorado’s Medicaid Program, and/or the Child Health Plan Plus (CHP+). The program also helps families on Health First Colorado (Medicaid) and/or CHP+ understand their benefits and find medical, dental, vision, mental health, and specialty services

Communities That Care (CTC) aims to prevent problems in youth before they develop. It is a cooperative, community-driven process that strengthens and supplements existing prevention work to reduce alcohol and substance use, violence, crime and other problematic behaviors.
